You can return the form by email (just cut & paste) to Pam Laricchia or if you prefer, you may print the form and snail mail it to Pam Laricchia, 9716 Wellington Road 22, Erin, Ontario, Canada, N0B 1T0.

Payment may be made through PayPal using the Buy Now button below, or by cheque by mailing it to the above address - please ensure your name is prominent if you registered by email so I can easily match it with your registration.
